Organization Ontario March of Dimes
Organization Description
Ontario March of Dimes (OMOD) draws on more than 55 years of experience to serve people with physical disabilities of all ages and created March of Dimes Canada (MODC) in 2001 to share its knowledge and expertise across the country. We are the largest community-based rehabilitation and advocacy organization for people with physical disabilities in the province.  The organization operates programs and services through over 70 sites in Ontario, has control over three subsidiary non-profits, delivers government funded and donor funded services to over 40,000 consumers annually across the province and across Canada.
Duties / Responsibilities
  • In consultation with the supervisor develop and implement rehabilitation programs for consumers with ABI
  • Assist consumers to gain greater autonomy in areas such as activities of daily living, cognitive and problem solving skills and community orientation and integration.
  • Obtain information and prepare reports or case histories
  • Reporting responsibilities, ie) progress notes, IRP, Incident reports, data collection
  • Assess client’s relevant skill strengths and development needs
  • Conduct follow-up assessments
  • Develop, coordinate and implement the delivery of specific services within the community
  • Provide suicide and crisis intervention
  • Liaise with other social services agencies and health care providers involved with clients
  • Develop service intervention models and outcome measures
  • Participate and provide training as required
Requirements / Qualifications
  • Post Secondary Education in Social Services or equivalent experience
  • Crisis Prevention Institute (CPI) training, First Aid Certificate
  • Experience in the field is an asset
  • Ability to speak, read and write English
  • Experience with Microsoft Office
  • Good interpersonal and communication skills
  • Must be available to work flexible hours
  • Demonstrated team building skills
  • Valid Driver’s License and access to reliable vehicle
